SCCo Damage Assessment Application (Beta)

The Santa Clara County Damage Assessment application is a phone-based app that is used to collect injury and damage information after a major event such as an earthquake. It should only be used in Santa Clara County.

Installing the app on your phone is a two-step process:

  1. Download the Survey123 for ArcGIS app from either the Google Play Store or the Apple Store.
  2. Download the SCCo Damage Assessment Field Survey form that runs in the Survey123 app.

Note: this is the beta test version of the app.  In case of a real emergency, do not use this app for collecting damage assessment information.  Any data collected and submitted at this time is not analyzed in any way.

Survey123 for ArcGIS

Download the Survey123 for ArcGIS app using one of the links below, depending on the type of phone you are using.  Note:  once you download the app, do not run it until after you have installed the Damage Assessment form.

Los Altos Preliminary Safety Assessment Form

This is the Los Altos-specific Preliminary Safety Assessment form.  This is an add-on to the Santa Clara County's Outpost packet message manager program.  You must install that package from the County's website first.  Once you have done that, you can download and run this installer.  See the County's page at https://www.scc-ares-races.org/data/packet/client-software.html
