The Santa Clara County Damage Assessment application is a phone-based app that is used to collect injury and damage information after a major event such as an earthquake. It should only be used in Santa Clara County.

Download the Survey123 for ArcGIS app using one of the links below, depending on the type of phone you are using. Note: once you download the app, do not run it until after you have installed the Damage Assessment form.
This is the Los Altos-specific Damage Assessment Summary form. This is an add-on to the Santa Clara County's Outpost packet message manager program.
